Request for Pre-Approval

Use this form to request pre-approval for purchases funded by the SOID UCS grant and/or items featuring UCS, Unified Sports, or Special Olympics Idaho names, logos, or branding.

Pre-approval is required for the following:

• The design and purchase of all apparel and uniforms.

• Any items featuring UCS, Unified Sports, or Special Olympics names, logos, or branding.

• Any purchases for which you plan to request reimbursement through UCS.
